Understanding the Heat Stress Spectrum
Heat stress is not a single condition. It progresses through stages, from mild heat stress through to a genuine medical emergency, and recognising which stage a worker is in determines how urgently you need to act.
Heat cramps are the mildest stage, involving painful muscle spasms, usually in the legs or abdomen, caused by fluid and electrolyte loss through sweating.
Heat exhaustion is more serious and involves heavy sweating, weakness, headache, nausea, dizziness, and a fast but weak pulse. The body is still able to regulate its temperature at this stage, but only just.
Heat stroke is a genuine medical emergency. It occurs when the body loses the ability to cool itself and core temperature climbs rapidly, often above 40 degrees. Left untreated, it can cause organ damage or death within a short window.
Recognising the Warning Signs
The signs of heat exhaustion are usually the last clear warning before a worker progresses to something far more dangerous. Watch for heavy sweating that suddenly stops or reduces, pale or clammy skin, muscle cramps, fatigue and weakness beyond what the work would normally cause, headache, nausea or vomiting, dizziness, and a rapid but weak pulse.
Heat stroke presents differently and should be treated as an emergency the moment you see it. Signs include a high body temperature, skin that is hot and either dry or unusually flushed, confusion or slurred speech, agitation or irrational behaviour, a rapid and strong pulse, seizures, and loss of consciousness. A worker who was previously sweating heavily and suddenly stops sweating altogether while still showing signs of overheating is a particularly serious warning sign.
First Aid Response for Heat Exhaustion
If you suspect a worker has heat exhaustion, act promptly even though it is not yet an emergency:
- Move the worker to shade or an air conditioned space immediately
- Have them lie down with legs slightly elevated
- Remove any unnecessary clothing or PPE that is trapping heat
- Cool the skin with wet cloths, a fan, or by sponging with cool water
- Offer small sips of water if the worker is fully conscious and able to drink safely
- Monitor closely. If symptoms do not improve within thirty minutes, or if they worsen at any point, treat it as a heat stroke emergency and call triple zero
First Aid Response for Heat Stroke
Heat stroke requires an immediate emergency response. Call triple zero straight away, then begin cooling the worker as aggressively as possible while waiting for paramedics to arrive:
- Move the person to shade or a cool area immediately
- Remove excess clothing
- Cool the body rapidly using whatever is available, wet towels, a hose, cool water sponging, or immersion if practical
- Apply cold packs or wet cloths to the neck, armpits, and groin, where blood vessels sit close to the skin and cooling is most effective
- Fan the person continuously while cooling
- Do not give fluids by mouth if the person is confused, drowsy, or not fully alert, due to the risk of choking
- Monitor breathing and responsiveness closely until emergency services arrive, and be prepared to begin CPR if they become unresponsive and stop breathing normally
Every minute of delay in cooling a person with heat stroke increases the risk of permanent organ damage, so do not wait for symptoms to worsen before calling for help.
Your WHS Obligations as an Employer
Under the model Work Health and Safety framework, a Person Conducting a Business or Undertaking has a legal duty to manage the risks of heat stress, whether outdoors in direct sun or indoors in a hot environment. Safe Work Australia's guidance on working in heat, alongside the Code of Practice for Managing the Work Environment and Facilities, expects employers to identify heat as a hazard, put controls in place, and consult with workers on how the work can be managed safely in hot conditions.
In practical terms, this generally means scheduling demanding physical work for cooler parts of the day, providing shade and rest breaks, ensuring drinking water is always accessible, and having a clear plan for what to do if a worker shows signs of heat related illness. Several state regulators have issued penalties and improvement notices to businesses that failed to manage heat risk adequately, so this is genuinely enforced rather than a box ticking exercise.
Where a First Aid Kit Fits In
It is worth being upfront about this. A first aid kit is not the primary tool for managing heat related illness. Prevention through shade, hydration, and scheduled rest breaks does far more than any kit ever could, and a genuine heat stroke emergency needs paramedics, not first aid supplies. That said, a well stocked kit still plays a supporting role, particularly a thermal blanket for after care once a worker has stabilised, and general wound and burn care in case the incident is compounded by another injury on site.

Heat stress moves fast, and the gap between heat exhaustion and a genuine medical emergency can close in a matter of minutes on a hot Australian worksite.
